From the main square of Montepulciano (Piazza Grande), head southeast on Via di Voltaia nel Corso. Continue straight for about 700 meters until you reach the intersection with Via di S. Biagio. Turn left onto Via di S. Biagio and follow it for approximately 1.5 kilometers. The Sanctuary of the Madonna di San Biagio will be on your right at Via di S. Biagio, 20.

If you are near the Montepulciano Fortress (Fortezza di Montepulciano), exit the fortress area and head down the hill towards Via del Lago. Once you reach the end of Via del Lago, turn left onto Via di S. Biagio. Continue straight for about 1 kilometer, and you will see the Sanctuary of the Madonna di San Biagio on your left at Via di S. Biagio, 20.
